I followed the palliative care pharmacist and observed her activities over 2 days. I was introduced to the palliative team (Doctors, nurses, allied health and support staff) and got an understanding of their roles and how they interact with each other. I also had the opportunity to visit a patient in the community at their house as well as observe a medication history for a newly admitted patient in the hospital and discharge for a patient going home. I also met family and carers for these patients. All of these patients had different needs with medications that the pharmacist could then report back to the doctor.

I was lucky to observe such a large variety of interactions during my placement.  In the future, I will know how to refer and what practitioners are available to support.
