Tara Leister – St John of God, Berwick

Completing my placement within the community palliative care team allowed me to achieve my goals around understanding the scope of practice of the community team and improving communication between myself and the OT team which I hope will lead to a better working relationship between the hospital and community team and therefore a smoother transition for patients discharging home. Practically, I have developed confidence in my own communication skills around clearly communicating with clients’ and families. This placement also introduced me to different outcome measures that will help in my role within the hospital in assessing clients’ function and equipment needs for discharge. I was able to complete a presentation that was circulated to the OT team within 2 hospitals to educate staff on our role within palliative care.
