  • Your name (as you would prefer it to appear on the PEPA website). *If your activity is part of a PEPA Aged Care placement, please include the name of your team or working group here.
  • Provide the name of your workplace - where you did the workplace activity.
  • Select the state or territory in which the placement was undertaken. If your activity was part of a PEPA Aged Care program, please select that option.
  • Provide a summary of your workplace activity (up to 200 word).
  • Please upload a photo that relates to the workplace activity. This can be a photo of you, your workplace or of the activity or initiative that came out of your PEPA placement experience.

  • Provide any supporting documents (eg, presentations) in PDF format. For example, if you have a PowerPoint presentation you can save it as a PDF and upload here.
